"I was fine before semaglutide." Was she really?

A patient sat across from me last week, frustrated. "I was fine before semaglutide. Now I can't go for three days at a time."

I pulled up her chart. Three years of intermittent bloating complaints. A fiber supplement she'd started and stopped twice. A note from 2023: "patient reports straining, incomplete evacuation, 3 to 4 times per week."

She'd had this problem for years. She just didn't call it constipation until it got worse.

What the data shows

A study published this week in Medscape (10,146 patients on GLP-1 medications) found over 30% of GLP-1 users report constipation. But fewer than 4% developed it for the first time after starting the medication.

That gap tells you something important: for most people, GLP-1 did not create the problem. It exposed a gut that was already running slow.

Your colon was moving stool at a pace that felt manageable. GLP-1 slowed transit further. What was borderline became noticeable. What was occasional became constant.

Why the cause changes the fix

If you treat GLP-1 constipation as a side effect, you reach for a short-term solution: a stimulant laxative, a one-time dose of magnesium citrate, a weekend fast.

If you treat it as a motility pattern your body has had for longer than you realized, you address two things at once:

Restart the contractions

Your colon moves stool forward through rhythmic muscular squeezes called peristalsis. When transit was borderline-slow before GLP-1 and the medication slowed it further, those contractions weakened. Herbal motility support (ginger root, triphala, artichoke leaf) can stimulate that rhythm without the dependency risk of stimulant laxatives.

Keep stool soft while it moves
