2. Run a pre-mortem before you execute.
Don't wait for the post-mortem to learn why a project failed. Before kickoff, gather the team and assume the project has already failed — then ask why. The risks people are too polite to raise in a normal planning meeting come pouring out when you frame it as explaining a failure that already happened. Plan responses for the top three.
3. When things go wrong, communicate MORE, not less.

Elite leaders do the opposite — they raise communication frequency in a crisis, because silence is where stakeholder trust goes to die. A short, honest "here's what we know, here's what we're doing, here's when I'll update you next" beats a polished report that lands three days too late.
