Electrolyte Imbalance

Dehydration happens when you've lost more water than you've taken in.

With electrolyte imbalance, the concentration and ratio of minerals in your fluids is off — sodium, potassium, magnesium, chloride, etc — regardless of water intake. You can have completely normal fluid volume and still have an electrolyte imbalance, or you can be dehydrated with electrolytes that are technically still "in range."

That’s why advice like “just drink more water” or “just add salt” can miss the mark; dehydration and electrolyte imbalance are separate conditions requiring individualized correction.

Both too little and too much of any one electrolyte can disrupt fluid balance, nerve signaling, and muscle function, leading to afternoon headaches, brain fog, muscle cramping, and low-grade fatigue that doesn’t resolve with water alone. Chronic imbalance is easy to miss on a single lab draw if no one’s looking for the pattern.


Evidence-Based Tips for True Hydration

  1. Don't wait to feel thirsty. By the time you reach for the glass, mild fluid loss might already be underway. Researchers have found that monitoring thirst alongside two other simple signals — body weight and urine color — will give you a better idea of hydration status.
  2. Take note of dark urine. Urine color isn’t a perfect measure, but it is very accessible (especially considered alongside physical symptoms). Generally, we want pale yellow urine. If your pee is consistently dark in color, fluid intake might not be keeping up with losses.
  3. Be careful about salt. Sodium does help the body retain fluid, which is why oral rehydration solutions include it. But most of us already consume more sodium than we need. The trend of adding salt to your water can raise your blood pressure over time, posing serious health risks. Speak to your clinician if you’re worried about sodium loss from endurance exercise or illness.
  4. Choose mineral-rich foods. You lose potassium and magnesium along with sodium when you sweat. Boost these minerals through nutrition with leafy greens, avocado, bananas, coconut water, and beans.
  5. Spread water consumption throughout the day. This gives your kidneys time to actually absorb and use the water before excreting it as urine. It also keeps plasma osmolality and electrolyte concentrations more stable, avoiding the brief dips in sodium that can happen when a lot of water is consumed in a short window.

True hydration requires steady, unglamorous consistency, informed by what your body is actually losing — not by what's trending.

BUN/Creatinine Ratio

The BUN (blood urea nitrogen) to creatinine ratio is a simple calculation from two markers already on most standard metabolic panels, but rarely explained to patients. It can offer an early clue about your hydration status.

BUN reflects how efficiently the kidneys clear nitrogen waste. It tends to rise disproportionately relative to creatinine when the body is running low on fluid, since the kidneys reabsorb more water and urea together under those conditions. Creatinine is a steadier marker of kidney filtration that doesn't shift as easily with hydration alone.

Why we look at it: A BUN/creatinine ratio on the higher end of normal — especially alongside symptoms like afternoon headaches or fatigue — can indicate chronic, mild dehydration.
